There are two ways to help ensure that your health care wishes will be respected:

  • HEALTH CARE PROXY
  • LIVING WILL

Both documents indicate what life support treatments you accept or refuse in the event you become incapacitated. The Health Care Proxy form provides instructions and also designates another person (usually family) to make health care decisions if the patient can’t do so. The Living Will is a document you sign in advance which provides specific instructions on health care treatment. Unlike the Health Care proxy, a Living Will does not appoint anyone to make decisions.
